한국의 통합돌봄 정책 재정 확대 필요성과 공공기관 중심 운영 방안

본 연구는 한국의 통합돌봄 정책 현황, 재정 문제, 그리고 효과적인 지역사회 돌봄 제공 방안을 분석하였다. 고령화, 저출산, 1인 가구 증가 등 인구 구조 변화로 인해 전통적 가족 중심 돌봄 체계는 증가하는 수요를 충족하기 어렵다. 2026년 중앙정부 통합돌봄 예산 914억 원은 시민사회 요구 예산 2,132억 원의 43% 수준에 불과하며, 이는 정책 목표 달성에 심각한 제약을 초래한다. 방문진료·재택의료 및 구강돌봄 사례 분석을 통해 공공 중심 체계 구축, 전문 인력 확보, 통합 관리 체계의 필수성을 확인하였다. 본 연구는 중앙·지자체 차원의 재정 확대, 공공기관 역할 강화, 돌봄 노동자의 노동권 보장, 지역 맞춤형 서비스 운영을 정책적으로 제언한다. 통합돌봄은 단순한 복지 지출이 아니라 사회적 인프라 구축과 경제적·사회적 순환을 촉진하는 생산적 투자로 인식되어야 하며, 공공 책임성과 지역사회 중심 운영을 통해 국민 모두에게 존엄한 삶과 지속 가능한 사회 재생산을 보장할 수 있다.

This study examines Korea's integrated care policy, focusing on its financial challenges and strategies for effective community-based care provision. Rapid population aging, low fertility rates, and increased single-person households have made traditional family-centered care systems inadequate for meeting growing care demands. The 2026 central government budget allocation of 91.4 billion KRW represents only 43% of the 213.2 billion KRW requested by civil society organizations, severely constraining policy implementation. Case analyses of home-visit medical services and oral care programs demonstrate the necessity of public-sector-led systems, professional workforce allocation, and integrated management frameworks. This study recommends financial expansion at central and local levels, strengthened public institution roles, labor rights protection for care workers, and region-specific service delivery. Integrated care should be recognized not as welfare expenditure but as productive investment promoting social infrastructure development and economic-social circulation, ensuring dignified living standards and sustainable social reproduction for all citizens through public accountability and community-centered operations.
