자궁경부암 예방을 위한 국가 공중보건체계 현황 분석 및 개선방안

자궁경부암은 인유두종바이러스(HPV) 감염으로 인해 발생하는 여성암으로, 예방접종과 정기검진을 통해 예방 가능한 질환이다. 본 연구는 자궁경부암의 병태생리, 임상증상, 진단 및 치료방법을 고찰하고, 국내 국가예방접종사업과 국가암검진사업을 중심으로 현행 공중보건체계를 분석하였다. 의료자원의 개발, 자원의 조직화, 의료서비스 제공, 재정적 지원, 정책 및 관리 측면에서 현황을 파악하고 문제점을 도출하였다. 개인, 가족, 집단 수준에서 HPV 감염 및 예방접종에 대한 인식 부족, 검진 참여율 저하, 지역 간 의료격차, 예방접종-검진-치료 연계 부족 등의 문제가 확인되었다. 이를 해결하기 위해 건강교육 강화, 통합관리체계 구축, 의료서비스 접근성 향상, 재정 지원 확대, 정책 개선 등의 개선방안을 제시하였다. 특히 HPV 예방접종 확대와 예방교육 강화가 가장 우선적으로 시행되어야 할 1차 예방 전략으로 강조되었다.

Cervical cancer is a female cancer caused by human papillomavirus (HPV) infection and can be prevented through vaccination and regular screening. This study examined the pathophysiology, clinical symptoms, diagnosis, and treatment methods of cervical cancer, and analyzed the current national public health system centered on the national HPV vaccination program and national cancer screening project in Korea. Current status was analyzed in terms of medical resource development, resource organization, medical service provision, financial support, and policy management. Problems were identified at individual, family, and community levels, including lack of awareness about HPV infection and vaccination, low screening participation rate, regional medical disparities, and insufficient linkage between vaccination, screening, and treatment. To address these issues, improvement strategies were proposed, including strengthening health education, establishing an integrated management system, improving access to medical services, expanding financial support, and improving policies. In particular, expansion of HPV vaccination and strengthening of preventive education were emphasized as the most prioritized primary prevention strategy.
